Berth collects information from you for the primary purpose of providing quality health care.

We require your personal details and medical history (including information from other treating medical practitioners, the Australian Immunisation Register and the Cervical Screening Register) so that we may properly assess, diagnose and treat illnesses and be pro-active in your health care.

 We may use the information you provide in the following ways:

- Administrative purposes in running our medical practice.

- Billing purposes, including compliance with Medicare and Health Insurance Commission requirements

- Disclosure to others involved in your health care, including SJOG Hospital and treating medical practitioners outside this medical practice. This may occur through referral to other doctors, or for medical tests and in the reports or results returned to us following the referrals.

We will not use your information for any purpose except these without discussing this with you first.
